Registered Nurse - Geriatrics

United States - Full Time

We are seeking Registered Nurses (Geriatrics) to join our teams across the US supporting the Department of Veterans Affairs. RNs provide and coordinate patient care on-site at a VA facility. They also provide education to patients and the public about illnesses, diseases, and health conditions. They're licensed medical professionals who work with patients, doctors, and other healthcare workers. RNs provide a high level of direct care to patients.

Key Responsibilities:

  • Documents assessment and interventions by policy and practice standards (CPRS)
  • Conducts bedside, patient-centered rounds utilizing the SBAR tool and all transition of care
  • Mentors and precepts new staff and students.
  • Crisis intervention for a unique population
  • Completes post-Emergency Department (ED)/Urgent Care and post-discharge phone calls within 2 business days, and documents results in patient's medical record.
  • Actively manages a high-risk registry which includes patients with elevated HgbA1C, hypertension, elevated LDLs, frequent hospitalizations/admissions, frequent visits to the ED/Urgent Care area, and those who are considered, by the team, to be at high risk for injury.
  • Collaborate with the Team (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven care plan for life.
  • Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's personalized health plan/goals.
  • Manages triage, telephone triage, and scheduled and unscheduled patients.
  • Executes nurse-driven clinics for appropriate patient populations to support positive health outcomes for veterans.
  • Promotes cost-effective use of resources and adopts effective least-expensive strategies expected in non-face-to-face approaches (including Telehealth).
  • Keeps patient's provider and appropriate case manager aware of patient progress, issues, and/or problems.
  • Screens patients for social service, home care, and other community care needs; coordinates or makes referrals as appropriate and seeks consultation when indicated.
  • RNs will be required to use an x-ray machine.

Experience Required:

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience:

  • Operative C-arm x-ray machine, wearing of lead protective apron, good communication skills, good assessment skills, Triage Veterans with pain issues, work in spinal procedure suite

Education Required:

  • Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
  • OR The completion of coursework equivalent to a nursing degree in an MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current BLS certification.
  • Current American Heart Association Basic Life Support (BLS) is preferred

Physical Requirements:

  • Work is normally performed in a typical interior/office work environment.
  • Work involves sitting and standing for prolonged periods.
  • May require bending, stooping, and lifting to 15 lbs.

Other Special Qualifications:

  • US Citizen or Permanent Resident Card is required.
  • Must relocate to the project site for the duration of the contract.
  • Pass a Child abuse and neglect check.
  • Pass/possess VA Public Trust Clearance
